چکیده

122 A 52-year-old patient with a history of rheumatic mitral stenosis was followed periodically at the cardiac outpatient clinic for dyspnea that eventually progressed to dyspnea on minimal exertion. The mitral valve had a score of 8 on echocardiography, basically because of valve thickening and diminished mobility, with minimal calcification. The anatomy of the valve permitted treatment with percutaneous mitral valvuloplasty, and we chose to perform the procedure guided by three-dimensional real-time echocardiography. The images show the mitral valve as seen from the left atrium. The baseline study showed severe stenosis secondary to rheumatic fever (an area measuring 0.6 cm2), with thickening of the leaflets and fusion of both commissures (Figure 1). After initial inflation with a 26-mm Inoue balloon for a body surface area of 2.2 IM AG E S I N CA R D I O L O G Y

Intracardiac ultrasound-guided percutaneous mitral valvuloplasty.

Intracardiac echocardiography (ICE) is a relatively new method of ultrasound images useful during cardiac percutaneous interventional procedures. The first human experience with this method was published in 2000 and, since then, several original trials have enrolled this useful method, proving similar and more useful than transesophageal echocardiography in percutaneous treatment of several con...

Intracardiac echocardiography--guided percutaneous mitral balloon valvuloplasty.

Percutaneous mitral balloon valvuloplasty (PMBV) was initially described by Inoue in 1984 as a novel percutaneous technique for the management of mitral stenosis. Intracardiac echocardiography was initially used in the 1980s but was not universally accepted due to its high-frequency transducers and problems with steerability and manipulation. In the 1990s technical improvements led to more gene...

Left atrium reverse remodeling in patients with mitral valve stenosis after percutaneous valvuloplasty: a 2- and 3-dimensional echocardiographic study.

INTRODUCTION AND OBJECTIVES The left atrium is clinically relevant in patients with mitral valve stenosis. The objective of this study was to analyze the effects of percutaneous balloon valvuloplasty on left atrium volumes and the left atrium emptying fraction in symptomatic mitral valve stenosis patients using 2-dimensional and real-time 3 -dimensional transthoracic echocardiography. METHODS...

Three-dimensional echocardiography in mitral valve disease.

Three-dimensional echocardiography offers great promise for improving the understanding of the mitral valve anatomy, function, and pathology. It may have important implications for medical or surgical management of different mitral valve disease. In this article we provide an overview of the three-dimensional anatomy of the mitral valve. Based on the studies using three-dimensional echocardiogr...

Role of echocardiography in percutaneous mitral valve interventions.

Intraprocedural imaging continues to evolve in parallel with advances in percutaneous mitral valve interventions. This didactic review uses several illustrations and rich intraprocedural videos to further describe and demonstrate the role of the most up-to-date echocardiographic and advanced imaging technologies in the patient selection and intraprocedural guidance of percutaneous mitral valve ...
